Service coordination is available to adults, youth and children. It is the active assistance offered to individuals with developmental disabilities as they navigate the community and various service systems in pursuit of the necessary and desired services and supports that will assist them in achieving and/or maintaining their personal goals. Medicaid Service Coordination (MSC) is a Medicaid state plan provided through the Office for People With Developmental Disabilities (OPWDD), and is eligible to individuals with developmental disabilities who receive Medicaid. MSC is provided by qualified service coordinators and uses a person centered planning process. The service coordinator works in a partnership with the consumer, individual, family and/or advocate to develop, implement and maintain the consumer’s Individualized Service Plan (ISP). This plan describes who the consumer is, his or her strengths, capacities, needs and valued outcomes, and the supports and services he or she wants to achieve these outcomes.
